patients pertaining to services provided. We are proud to

Service Policy for Your New Restoration
Crown, Bridge, Veneer, White (Bonded) Filling
Your new restoration is made using the finest and most durable
material available in dentistry. We have learned that despite
our best efforts, restorations can fail for a variety of reasons.
These include: new decay, breakage from excessive bite forces,
grinding of one's teeth, or simply biting down on a hard object
(such as a fork, bone, ice, hard candy, etc.). Understand,
most restorations are utilized by the patient several times
per day and hold up well for decades under normal conditions.
In fairness to both the patient and the doctor, the following
schedule outlines how Dr. Seasholtz will address fees in the
unlikely event that your restoration should require replacement
within the first five years of service. Although the Board
of Dentistry recommends restorations should last at least
four years, Dr. Seasholtz utilizes the Five Year Benchmark,
as this is the typical period needed to elapse by insurance
carriers before they will pay toward the cost of replacing
your restoration, based upon current fees.
FIRST YEAR OF SERVICE (from time of placement) - 100% Coverage
SECOND YEAR OF SERVICE - 80% Coverage
THIRD YEAR OF SERVICE - 60% Coverage
FOURTH YEAR OF SERVICE - 40% Coverage
FIFTH YEAR OF SERVICE - 20% Coverage
SIXTH YEAR OF SERVICE - 0% Coverage
As an example, if your crown requires replacement 18 months
after cementation, you will be in the second year of service.
If a new crown costs $1000 at current day fees, you will pay
only $200 to have it replaced and Dr. Seasholtz covers 80%
of the cost of replacement. "Year of Service" will
be determined by exact dates, specifically: Date of cementation
(permanent) to date of diagnosis by doctor or notification
by you (by phone or in-person) that you believe your restoration
was broken. While most dentists do not offer a service policy,
or warranty, Dr.Seasholtz provides this service policy in
order for his patients to know what to expect from the dental
care provided. Understand dentists incur much expense to provide
crowns for patients. For a service policy to be fair to
all concerned it is required that you have your dental restorations
evaluated by Dr. Seasholtz at least one time per year.
This will allow problems, which may fail the restoration,
to be corrected and keep your dental health care costs down
over time.
